There is one per each of the five (four?) major Temple Cities. Mazdamundi rules Hexoatl.

Is there any official source on the other four (or is it three)? At the very least name-city pairings?

Or do I have to (get to) make them up?

I believe they are unnamed, But I assume they rules the other active temples, (see map).

There are 4 temple cities, including Hexoatl.
I would not think that there is an official source on the slann of the other three. My thinking for this is that there is more than one Slann per temple-city. On page 22 of the army book (the bit about the cities) it says:
"At the heart of every city are the pyramid temples. Large and majestic, these stepped ziggurats loom over everything. At each of their summits, far above the world, sit the most ancient of the city's Slann Mage-Priests."
Wouldn't that mean that there are other, less ancient Slann? It also mentions the younger Slann later in the paragraph.
So yes, you can make up your own Slann :)

I remember reading somewhere that there are 3 living 2nd gen. Slann, although the Slann who rules my Temple-City is second generation but most Slann think he's dead

Officially GW has four second generation Slann: Mazdamundi of Hexoatl, plus a second gen Slann in Itza, Tlaxtlan and Xlanhuapec. I like to ignore GW fluff periodically so if you want a lost second generation Slann more power to you. As far as I'm aware, Mazdamundi is the only second generation Slann with a name. I decided to name the ruler of Itza Chuqxlata. He is half-awake and kind of a jerk to my Klodorex crew. Mazdamundi is just going to assume the lesser Slann will follow his lead, but Chuqxlata is not convinced so he directed his subordinates to constrain Klodorex.

I have assigned the name Xixlatel to the ruler of the Xlanhuapec. He will very slowly become a player in my stories. The ruler of Tlaxtlan can remain nameless and in catatonic state until I can think of a story purpose for him.

As for Klodorex, Merestar is of the Third Generation. Desserex is of the Fourth Generation. Laershin and Slann to be named later are of the Fifth Generation. Lord Trawlikshen of the Southlands is the only awake Slann of the Third Generation in the Southlands and is nominally in charge of all the Lizardmen Temple cities there.

I believe the 6th edition book stated 5 second generation were alive. Offc one might have buggered off in the meanwhile!

From a fluff-wise point of view, i'd think that there are the 4 rulers of the major temple cities, and the "mad slann" that went out to make havoc in the 7th edition book, since it could seem like a 2nd would carry enough weight to actually bring an army and just leave.

dunno if they changed the amount later though, or rather, i can't remember

ooh, and i had forgotten the powers of fluff has already lead me to "birth" a slann that is at the very least 3rd edition, maybe older, (probably, but i don't see any need to put a number next to him) that was met by my krox / skink duo in "and the petals tasted like heaven"

On page 28 of Thanqol it says that all of the Second Generation Slann except for Mazdamundi of Hexoatl and Adohl-Tehga of Tlaxtlan live in Itza. Then on page Then on page 36 it talks about Lord Huinitenuchli a Second Generation Slann defending Xlanhuapec.

Maybe he moved to the City of Mists to help? His bio paragraph goes on to talk about what he has done in the city over centuries. Bad GW, no biscuit!

I certainly don't feel obligated to rename Xixlatel to Huintenuchi if even GW can't keep their own stuff straight within an eight page spread.
